Assumption of Risk

I understand that the use of jet skis and personal watercraft involves risks including, but not limited to, drowning, high-speed collisions, propeller injuries, falls, exposure to harsh weather, and equipment failure. I voluntarily assume all such risks.*

Fitness to Participate & Substance Use

I certify that I am physically and mentally fit to operate or ride on a personal watercraft. I am not under the influence of alcohol, illegal drugs, or impairing medications. I understand that operating under the influence is dangerous, illegal, and may result in immediate termination without refund. I accept full financial responsibility for any damage caused under such conditions.*

Release & Indemnification

In consideration of being allowed to rent and operate personal watercraft, I release and hold harmless Cool Waves Rentals, its owners, staff, and affiliates from all liability for injury, death, or property damage. I agree to indemnify and defend them against any claims related to my actions.*

Responsibility for Damage

I agree to be financially responsible for any damage I cause to the equipment due to negligence, recklessness, or misuse.*

Governing Law

I agree this waiver is governed by the laws of the State of Oklahoma, and any legal action must be filed in Comanche County.*

Life Jacket Use

I agree to wear a U.S. Coast Guard-approved life jacket at all times while operating or riding on a personal watercraft.*

No Passenger Switching

I understand that passenger switching is not permitted once the rental begins, due to dock safety concerns. (Only applies to jet ski rentals).*

Arrival & Fuel Policy

I agree to arrive 30 minutes early, and I understand that fuel is not included in the rental. I will be charged for fuel used upon return. (Only applies to jet ski rentals).*

Acknowledgment of Understanding

I have read this waiver and fully understand its terms. I sign it freely and voluntarily.*

I am the parent/legal guardian of the minor(s) listed below. I give permission for them to participate and agree to all terms on their behalf.*
